Dear Parents/Guardians:
MEDICATIONS:
Please come pick up any as needed medication your child has in the clinic.
ALL medications not picked up by an adult will be disposed of on the last school day.
Please make arrangements to get that done in time.
Please pick up all Emergency medications and daily medications on the last day.
We do not send any medication home with students.
VACCINE RECORDS for 2018-2019 school year
If your child is attending Kindergarten next year.
Please turn in the vaccine records ASAP.
This allows time to get notification from the nurse and address any delinquent vaccines before the beginning of school.
We can not fully enroll students and put on a class list until we have complete up to date records prior to the first day. This is Texas law.

PREPARING TO START the NEW SCHOOL YEAR WITH Prescription MEDICATIONS:
We can not accept medication or administer medication at school until the Request to Administer Medication forms have been completed and signed by a DOCTOR licensed to practice in the USA and a PARENT or LEGAL GUARDIAN. We will also need Doctor and Parent signatures on any action plan for Asthma, Seizures or Anaphylaxis (food or insect allergy) medications. We need action plans to accompany medications for those diagnoses.
Please print these forms and have them completed before arriving to school to drop off the medication with the school nurse.
The School Nurse will be working starting on August 6th 2018
This is a good time to bring medications and forms before school starts.
Please note : ALL prescription Medications MUST have Doctor and Parent/Guardian signatures on the required forms BEFORE the medication can be brought to the clinic and before the medication can be administered.

UPDATE:
Medical Devices: Doctor’s note is required for use of medical devices on campus.
This includes but not limited to crutches, wheel chairs, canes, boots, etc.
We do not provide the medical devices.
Clinic request:
Our best practice recommendation especially during flu season is when any parent brings a small child into the clinic we are asking that they stay in your arms while in the clinic to help protect them from the communicable illness that could be on clinic surfaces.
